Thank you for your interest in working with us. Before scheduling your first session, we ask all new clients to complete a secure registration through our client portal. This allows us to gather the necessary information to better support your care.
The registration process is simple and secure, and should take only a few minutes to complete.

The registration forms will guide you through submitting your contact information, insurance details, identification, and payment information, as well as completing the necessary consent and intake paperwork.
All information is submitted through a secure and confidential system designed to protect your privacy.
What to Expect During Registration:
During the registration process you may be asked to:
• Provide basic contact information
• Upload your insurance card (if applicable)
• Upload a valid form of identification
• Add a credit or debit card to keep on file
• Complete intake and consent forms
